Primary Pulmonary Hypertension

56 year-old male with right ventricular strain on ECG. Nuclear ventilation/perfusion scan and CT angiogram were equivocal for pulmonary embolus. Pulmonary arteriogram was requested to definetively rule in/out pulmonary embolus.

 

Left main pulmonary arteriogram and..

 

 

right main pulmonary arteriogram demonstrate enlarged main pulmonary arteries with tortuosity and tapering of the distal branches. Pulmonary artery pressures were measured at 73/33 mm Hg with a mean pressure of 49 mm Hg.

(Normal PA pressures = 15-30 mmHg systolic, 5-15 mm Hg diastolic, 7-16 mm Hg mean).

Diagnosis = pulmonary hypertension.
